ABI REHABILITATION SERVICES​

Community Solutions specializes in assisting individuals with an Acquired Brain Injury to re-establish their lives in their own homes and communities. 1-1 Rehabilitation services and supports are available throughout the province of Ontario, up to twenty-four hours per day and seven days per week. Our team provides an essential, re-integration link between a hospital or clinical environment and a more natural setting. As a result, Community Solutions may become involved with the individual, immediately prior to or just after discharge from hospital, complementing the existing interdisciplinary team. This facilitates a smooth transition from one setting to the next and allows rehabilitation gains to be incorporated into the natural home, work, educational and recreational/ leisure environments. At that time, both the person and the family play a crucial role in the matching of Rehabilitation or Community Support Workers.

Community Solutions instills in our staff a genuine concern for the individual’s welfare, as well as a creative sense of teamwork and collaboration.
